Healthcare Concierge (Care Partner) – Remote Pharmacist

About the role

AllCare.ai is seeking a pharmacist to join its remote Healthcare Concierge team. As a Care Partner you will be the single point of contact for a portfolio of Assisted Living facilities in California, providing proactive, personalized support that goes beyond traditional call‑center interactions.

Key responsibilities

• Own a portfolio of California‑based Assisted Living facilities as the Primary or Secondary Care Partner.
• Perform medication reconciliation and coordinate pharmacy services for residents.
• Provide clinical judgment on drug‑related questions and ensure safe medication practices.
• Conduct outbound outreach: callbacks, proactive check‑ins, and follow‑through on open tasks.
• Collaborate with AllCare’s clinical, routing, and clinic teams to resolve issues before they impact residents.
• Maintain detailed knowledge of each resident and facility to deliver a highly personalized experience.

Required profile

• Licensed pharmacist with experience in medication reconciliation and pharmacy coordination.
• Strong customer‑service mindset; treats each interaction as a craft, not a queue.
• Ability to work independently in a remote environment and manage a defined portfolio of facilities.
• Excellent communication skills and a proactive, problem‑solving attitude.
